How to Start "Bad Blood" in Kingdom Come: Deliverance 2
Progressing through the main quest involving finding Mutt will eventually lead you to a clearing near Bozhena's hut. Henry will suggest checking with Bozhena. Speak to her about Mutt; she'll reveal her daughter Pavlena is missing. Agreeing to help initiates the "Bad Blood" quest.
Gather Information
Your investigation begins in Troskowitz. Speak with Bailiff Thrush, then Innkeeper Betty at the tavern. They'll provide crucial background information on Bozhena and Pavlena, shedding light on their ostracization from the village.
Next, find Woodcutter Dushko. He'll direct you to Roman's house. Note: you'll need lockpicking skills; save your game beforehand. Search Roman's house, examine Pavlena's basket, and return to Dushko for more leads. He'll point you towards a clearing, a favorite spot for the couple.
Search the Scene
Follow the blood trail from the clearing near Roman's house, down the hill towards the stream, then left past two large boulders. You'll discover Roman's body. Continue your search, speaking with Hogerd Hugo, then return to the Troskowitz farm and speak with Hired Hand Straw. A successful dialogue check with Straw will reveal information about Jakesh and the events surrounding Roman and Pavlena. You can then speak with Thrush to decide how to handle Straw before proceeding. Finally, head to the rocks south of Zhelejov to continue your investigation.
Confront Ota

Navigate around the large boulder in the objective area to find a hidden path. Climb the ladder to discover Ota and Pavlena. High Charisma is needed for successful dialogue checks. The following options will likely persuade Ota to release Pavlena: "What's your name?", "Let her go and I'll let you go.", "I'll put in a good word for you with His Lordship." Even if you succeed, Pavlena will likely kill Ota. Speak with Pavlena and return to Bozhena together. Bozhena will then request you kill Jakesh.
Kill Jakesh or Make Peace

The final choice: kill Jakesh or broker peace. Find Jakesh at his child's grave. You can choose to kill him, talk to him, or even kill Bozhena to end the quest.
Should You Kill Jakesh?
Killing Jakesh lowers your reputation in Troskowitz but significantly boosts it with Bozhena and Pavlena. You'll unlock a new dialogue option with Pavlena and receive a necklace, a gift from Roman.
Sparing Jakesh and achieving peace earns you 100 groschen from Jakesh. You can keep the money or share it with Bozhena, who will allow her and Pavlena to return to the settlement.
